I offer the best possible treatment up front in a detailed estimate. If the owner declines any of the recommendations, I note them on the estimate and in the patient's record. I then offer a "B" treatment plan with as many best options with some alternatives (such as SQ fluids when IV fluids and hospitalization are declined). I also ask the owner to sign the estimate, and an updated estimate if necessary, so that they are aware of their animal's treatment and diagnostic plan.
